I'll skip the first and go directly to the second. There are NO field labels on a form. There may be nearby text which a user may associate with the field mentally as a label, but there are no labels. Same with headings (apart from section labels) -- text of a particular size, weight and colour may look like a heading, but that's part of the user's mental process.
